Can you get gears for differential indexing?

It comes with 3 indexing plates for any combination you might need. The most important thing you need to do before using is take it completely apart including the chuck and deburr , clean and put in some good grease and oil, it is horrendously dirty inside, otherwise it seems of good quality.

Can I use this as a welding positioner turntable with a drill connected to it?

Yes you could absolutely use this as a welding position/turntable. If your mechanically inclined it would be simple to set it up with a motor and footswitch for welding

This can be used as a vertical OR horizontal table?

Yes. You can tilt the head 90 degrees. Make sure a verical table is sturdy. The rotating head is heavy.
